Driving Under the Influence (DUI)

If you’ve been arrested for driving under the influence (DUI) in Lake County, Illinois, an alcohol/drug evaluation is mandatory. This evaluation is a crucial step in assessing the extent of substance use and is required by law to move forward with your case.

What do I have to bring for a DUI evaluation appointment?
You must bring the following documents to your evaluation.

• Driver’s abstract for “court purposes” obtained from any Secretary of State facility
• Notice of Summary of Suspension and/or Sworn Report (a white piece of paper that was issued at the time of your arrest)
• Copies of all tickets issued at time of arrest
• Attorney’s contact information
• Appointment Fee

Once your evaluation is complete, your counselor will discuss with you any recommended next steps based on your evaluation.
Depending on the outcome, you may need to participate in additional education or treatment programs. At Nicasa, we offer a variety of services to help you meet these requirements, including:
