What do I have to bring for a DUI evaluation appointment?
You must bring the following documents to your evaluation.
- Driver’s abstract for “court purposes” obtained from any Secretary of State facility
- Notice of Summary of Suspension and/or Sworn Report (a white piece of paper that was issued at the time of your arrest)
- Copies of all tickets issued at time of arrest
- Attorney’s contact information
- Appointment Fee
What Happens After the Evaluation?
Once your evaluation is complete, your counselor will discuss with you any recommended next steps based on your evaluation.
